Extended Care Registration – Full Time Only Form

This form is only to be filled out if your student(s) intend to use our extended care program on a full time basis.

Do not use this form for the occasional use of the Extended Care Program. Before Care is $100 month for first student and $80 for additional students per month. After Care is $225 per month for first student and $200 per month for additional students. A $10 deposit is required and will be taken off your first months bill. Please call the office with your payment.
